Report Number: 129
Completed By: Ghorev
Opened By: Ghorev
Department: FLEET
Stardate: 54036.5
Ghorev added on 53838.5: I have been in communications with the Office of the Commandant, Starfleet Academy, regarding the severe disciplinary lapses of Cadet Moroko Torin. Though it is rare that a Cadet fails only the disciplinary and professional review of a Cadet Cruise and not the academic and skills portion, it does happen, and a set of guidelines exist, which I have been forwarded. Accordingly, I require three senior officers to volunteer to serve on a formal board of review. I will give further details in the addenda to this report, which will also serve to hold the board's deliberations and findings.
------------------------------------------------------------------------------
Ghorev added on 54036.5: I have received reply back from the Office of the Commandant, Starfleet Academy, advising that the official findings and recommendations of the Board of Review in the matter of Cadet Moroko Torin are being accepted, upheld, and published in their entirely. Orders for the Cadet to remain here on Station 419-Upsilon for a second cadet cruise with a transfer to the Science Department have been received and forwarded to CDT Torin. He has been assigned to the open Lab Technican billet in the Physical Sciences Section, where he will be supervised by LT Laco and LTJG Shamash. The thanks of the Commandant go to the review board, and as Chair of said board, I wish to extend my own thanks to LT Laco for the final compromise agreement in this matter. All personnel action in this case is executed effective immediately, and this report is closed and filed.

Stardate: 54042.4
Ghorev added on 54010.3: On Stardate 54009.0, USS Thomas Paine-C flew under my command as lead ship in Escort Wing 22-Foxtrot of the allied battle fleet engaging the giant artifact known alternatively as 'the Obelisk' and 'the Eater of Worlds'. LT Ivos, LT Idrani, LT Cristobal, LTJG Kepler, LTJG Medes, and ENS O'Carroll respectively handled the Engineering, Science, Ops, Tactical, Flight Control, and Mission Ops consoles for the engagement. The Saber-class ships USS Baden-Powell, USS Harrington, USS Cyclone, and USS Prodigy, and the Defiant-class Heavy escort USS Juggernaut flew subordinate to Thomas Paine in Escort Wing 22-Foxtrot, under Admiral Graves' 'order of battle', citing the greater experience of Thomas Paine's command crew and equating it to greater tactical superiority under the articles of Regulation 191, regarding leadership in coordinated fleet actions. The mission was a qualified success, with much follow-up pending. This report details that initial engagement.

Stardate: 54042.7
Kepler added on 54030.9: As per the orders of both Lt. Commander Park and Lieutenant Havaris, Eliza Roth is to be confined to her quarters pending Havaris' further consultation with Delegate Churas. There is to be a security detail posted at her door, security monitoring assigned to her room, with counselling and medical on on-site standby to aid in Roth's return to consciousness.
After suffering some manner of seizure, Roth was brought to the Infirmary whereupon she suffered a psychotic episode, assaulting Delegate Churas in her attempt to escape.
Please note that according to Starfleet records, she has a severe phobia to medical-related personnel and equipment and is very well versed in several martial arts techniques. The latter I can now personally vouch for.

Opened By: Havaris
Department: FLEET
Stardate: 54066.2
Havaris added on 53890.1: Once OERs for Ops are completed, we will begin another reshuffling of personnel in accordance with the test results and the new rank distribution within the department. As always, Operations welcomes officers of other departments should they wish to pursue administrational experience outside the scope of their initial training. Operations experience is an excellent opportunity to broaden your skills with a mind towards command, and is in the background of most commanding officers within the Fleet.
Currently, Operations has openings for a Junior Flight Deck Control Officer (jFDCO), a Junior Systems Manager (jSM), and a Junior Quartermaster (jQO). Systems Manager is an excellent post for engineers -- especially those with Core Systems experience. Quartermaster is an excellent step from Yeomanry or administrational Command Branch duties or Armory, and Flight Deck is an excellent post for Space Science or Tactical personnel.
Interested officers from any department -- including Operations -- must first acquire the consent of their Section Head and Department Head prior to relaying their interest to Lt. Havaris. An Ops SQT pursuant to the desired post must be passed in order for full consideration. Ensigns and Lieutenants of either grade eligible.
This report will remain open until the posts are filled and the redistribution of officers is complete. It will be updated as required.

Havaris added on 54066.2: Due to a lack of interest, this report is closed. One officer has expressed an interest in joining the department, and that has been handled internally.

Report Number: 354
Completed By: Javits
Opened By: Cristobal
Department: FLEET
Stardate: 54073.3
Cristobal added on 54030.1: Waking in my quarters on level 10, I discovered a strange and rather foul odor. I tracked the source of it to the third section of the level. Beneath the carpeting, several patches of the corridor's deckplating had turned a pasty-white, as if something were eating away at it. This continued into the fourth section, where the white patches were also approximately five centimeters up the bulkhead. Scans revealed that whatever the cause of the discoloration and the odor, it was spreading. Where the discoloration occured, the deck-plates were significantly weakened. Additionally, the order it put off was similar to an extremely heavy concentration of mildew, and could be harmful to humanoid respiratory systems with prolonged exposure. I contacted Operations and ordered an evacuation of level 10, and ordered all departing the level to proceed to the Infirmary. At this time, Engineering teams should report to level 10.
------------------------------------------------------------------------------
Javits added on 54073.3: Cleanup of levels nine and ten as well as replacement of all affected surfaces and furnishings has been completed. Final tests confirm that both levels and spaces between those levels as well as the affected hull plating are free of the phage. Those personnel whose personal effects were removed for disinfection and cleaning may retrieve them from secure storage located on level twenty-five. Complaints and damage claims are to be addressed to me.

USS Levy Rescue AAR

2004 May 07
|
Report Number: 404
Completed By: Ghorev
Opened By: Edwards
Department: CO
Stardate: 54076.3
Edwards added on 54045.3: With the consent of Lieutenant Havaris, I led a mission to rescue the downed crew of the USS Levy, who had crashed on an asteroid in a nearby asteroid field and were suffering from paranoid delusions. My team consisted of Lieutenant Ivos, Counselor Gellan, Lieutenant Darax, Doctor Vala and Ensign O'Carroll. We took the combat shuttlecraft Shoemaker. After getting underway, we used a set of sensor relays to scan the asteroid and ascertained that the downed crew was inside the asteroid, wearing their EVAs, and in the company of a human male of 130 years in age. One hundred of those had apparently been spent in stasis. We also ascertained that the Levy's cascading power failure had been stopped and was in the process of being repaired when the vessel had been abandoned. It wasn't long before we heard a signal coming from mining settlement on the asteroid, an SOS in morse code. When we arrived at the asteroid, scans indicated that the abandoned mining operation was littered with decaying corpses, and that a deposit of trihoxinal gas had been struck. Trihoxinal gas, according to Doctor Vala, is a hallucinogenic that causes paranoid delusions in humanoids. After being notified that the transporter's biofilter should eliminate the toxins from their bloodstreams, and that they would need medical attention on a biobed to make sure it was fully flushed, I ordered the team on the surface and the 130-year-old survivor beamed aboard. We entered the asteroid field and transported them aboard. Unfortunately, the transporter's biofilters failed mid-transport and the team came up paranoid and armed. Reacting quickly, Lieutenant Darax ran them through the transporter buffer a second time, filtering the toxin and their phasers. Then Doctor Vala and Counselor Gellan saw to the team while we returned home.
