Part I
Give Full name of officer being evaluated, their rank, their age, their species, their gender and their home planet (all covered in the SR so perhaps this step can be skipped though it looks professional to have it included.)
Part II
The name of the officer performing the evaluation for and their rank and position.
Part III
A description in the words of the evaluator of the officer's position and duties.

A) Character (Disposition of the leader: combination of values, attributes, and skills affecting leader actions.)
Starfleet Values (Comments mandatory for all "No" entries in part V a.)
1.Honor: Adherence to Starfleet's publicly declared code of values (yes/no)
